POSITION SUMMARY
The purpose of the Peer Services Manager role is to develop and foster seamless patient access to internal levels of care and to needed community resources. This position is vital in coordinating with all levels of care within the Fora Health treatment continuum, ranging from the patient resource center to withdrawal management, residential, outpatient to Fora Health alumni. This position ensures patients needs are identified and met at all points of access and that patients are provided opportunity to access community resources ranging from entitlement benefits to transportation to employment opportunities.
ESSENTIAL JOB FUNCTIONS
Adult Residential Milieu Operations
- Leads peer mentor services operations for all levels of care, and coordinates with Residential Program Manager and RA Supervisor to deliver a high-quality residential treatment program that ensures peer mentor support for housing, physical health, and recovery resources.
- Coordinates pro-social recovery activities within residential and outpatient levels of care, which include both internal and external recovery events.
- Provides a safe, stable and nurturing environment for patients so they may together create a community of healing from addiction while developing the skills and resources to maintain recovery.
- Maintains collaborative, consultative relationships with clinical and medical leadership for coordination of patient services.
- Collaborates with internal departments in order to provide seamless patient transitions through the continuum of care.
- Hiring, recruitment, training, and retention of staff. Oversee onboarding and training processes for all peer mentors.
- Oversee the Peer Services Supervisor and peer mentor teams. Ensure there is ongoing quality improvement.
- Assure patients have access to all resources, materials and supplies of activities of daily living through case management or referrals that are maintained by the peer mentor team (s).
- Collaborate with Peer Services Supervisor to ensure that the team meets program and professional standards for providing quality care and meeting budget and revenue goals.
- Support consistent operating hours for the Patient Resource Center and coordinate with community partners to provide on-site access to resources.

- Remain up-to-date on community resources available and offer guidance to all programs on how to support patients in accessing community resources.
- Maintain ongoing partnerships with H&I and coordinate in-house meeting opportunities for patients.
- Provides leadership and supervision to the Peer Services Supervisor and peer mentor team(s)
- Assures the treatment team shares organizational values and helps develop and maintain a style of working together which enhances productivity, creativity and staff satisfaction across the interdisciplinary teams.
- Partners with the supervisor team to develop policies and procedures to expand peer mentor services at Fora Health across the full continuum of care.
- Demonstrates a commitment to increasing the quality of the services delivered, by effectively addressing the needs of the populations served and incorporating evidence based practices to the models of care.
- Obtains (if applicable) and maintains current CRM, PSW, or PSS and other applicable credentials/licenses.
- Provide direct service, to include outreach, peer support and connecting individuals to community resources to support the team in meeting enrollment and productivity benchmarks.
- Develop workflows and documentation within current EHR to track transitions through level of care and measure outcomes of needs addressed by peer teams.
- Develop workflow and community partnerships to ensure patient access to outside medical care from within all levels of care.
- Is an active member of the Residential Leadership Team to ensure that peer mentor services are integrated into residential in a seamless manner. This includes participating in the after-hour on call rotation.
- Collaborate with other Fora Health programs to identify and meet their needs.
- Work closely with all adult service teams to coordinate support of patients.
- Ensure that workflows are developed and communicated to all Fora Health programs to ensure ease of access to services.
- Train and oversee teams on safe and effective outreach techniques.
- Work closely with Withdrawal Management, Admissions, Adult Residential, and Adult Outpatient to identify patients who are in between levels of care and would benefit from outreach and engagement.
- Provide educational sessions and seminars for patients and their families on the nature of addiction as a disease and levels of care offered at Fora Health.
- Support patients in navigating the resources and services available to them to support their recovery and their family.
- Identify barriers to access and treatment retention for patients and coordinate with all teams to address barriers.
Leadership Responsibilities
- Stay current with agency changes in programs, policies, or procedures, and communicate this information to direct reports, and assure staff are integrating this information into daily practice.
- Follow through with necessary progressive discipline in collaboration with the Human Resources team.
- Ensure financial stewardship of the peer mentor team(s), to include tracking grant funding when applicable, oversight of productivity standards, and monthly budget monitoring.
- Ensure appropriate and therapeutic interactions occur among the patients and peer mentor team members.
- Serve as a role model for staff and patients by demonstrating ethical behaviors, effective communication and suitable problem-solving skills.
- Directly supervises Peer Mentor Supervisor.
- Ensures peer mentor team receives consistent skills training and develops additional training opportunities based on needs of the team.
- Assists in recruiting, hiring and retaining staff and is appropriately credentialed and trained.
- Promptly completes reviews for direct reports on a first 90-day, annual and as-needed basis.
- Ensure that supervisors are providing a minimum of two supervision sessions to their direct reports on a monthly basis.
- Maintains supervision logs and monthly staff meeting records.
- Oversees driving training and driving eligibility for the peer mentor team (s). Partners with Facilities Manager for regular maintenance of Fora vehicles.
- Special projects and other duties as assigned
EDUCATION AND EXPERIENCE REQUIRED
- Minimum of 2 years of supervisory experience (behavioral health field preferred)
- Must posses an active Certified Recovery Mentor (CRM) certification
- Proficient in Microsoft Office applications
- Communicates information effectively in writing and verbally
- Strong organizational skills and attention to detail
- Displays initiative and problem-solving skills
- Intermediate knowledge of substance-use disorders
- Knowledge of 12-step principles
- Intermediate understanding of trauma-informed care
- Working knowledge of ASAM and DSM 5 Substance Use Disorders
- Ability to be approved as a company vehicle driver by Fora Health
- For employees with a history of substance use disorder, Fora Health’s policy requires certification of a minimum of two years of sobriety for this position.
- Ability to pass a pre-employment or for cause drug tests.
- Ability to pass DHS criminal background check.
- Position will require some amount of regular connection to work via cell phone to answer questions and respond to internal and external contacts.
- This position generally works in an indoor office environment with some travel between sites or attending community coordination meetings.
- Position requires some schedule flexibility, which may include overtime, evening or weekend hours, to be communicated by supervisor.
- Possible exposure to communicable diseases which can encompass a variety of illnesses and infections, including tuberculosis, Hepatitis A, B, and C, and HIV.
- Possible exposure to upset, angry, traumatized, or emotionally disturbed adults, adolescents, and families.
- Exposure to potentially hazardous cleaning chemicals, personal protective equipment provided.
